The following article shows how to create a subscribe webform using formmail.pl written by Matt Wright.
This PERL-script is installed on many webservers.

HTML soucecode for a HTML subscribe form using fommail.pl installed on your web server:

The blue lines are specific code for FormMail. If you use a different script, you need to adjust these lines. 
The green lines are universal code, used to add 2 fields (name and email) to our form.

 

Now save the file to your webserver and click on "Submit".
